For Problems: Please include details to assist us in diagnosing your problem. Your request may be delayed if additional information is needed to direct or resolve your problem. Include things such as:
  1. For login/account problems, your username or account number(s) and the name of the system to which you are trying to connect.
  2. What you were doing when the problem occured.
  3. What you were trying to do when the problem occured.
  4. Additional details that may help to replicated the problem. For example:
    • Computer name/location/IP address (especially for public or department lab computers).
    • How the computer is connected to the internet.
    • OS name and version.
    • Program(s) used when the problem occurred.
    • For a failure, were you successful in the past? On the same computer? On another computer?
    • Have there been recent changes to your system? What changes?
  5. Any error message you received (cut and paste it if possible).
Please do not include sensitive data, such as SSNs or passwords.
